Registration Fee: $450
Capacity: Limited to 22 participants on a first-come, first-served basis with proof of eligibility and registration.
The registration fee includes all supplies, refreshments, lunch, and parking. Participants must wear scrubs and bring their own safety glasses, toothbrush, and toothpaste.
Cancellation Policy: No cancellations after November 21, 2026.

To be eligible for the Coronal Polishing Certification Course, participants must provide the following documentation:
Documentation must be submitted electronically during registration or by email or fax. Participants who do not submit the required documentation within 10 days of registering will automatically be removed from the course.
Participants must also complete a Sinclair Medical History Form. This form will be sent with the email confirmation after pre-registration and receipt of all required documentation.
It is highly recommended that participants receive a complete scaling or oral prophylaxis within three months of the course date.
