Student Organization Forms
The following forms are Word documents that may be completed on the computer and printed. The sample constitution is to be used as a guideline for your organization's constitution, and we recommend that you stick with this format as closely as possible. Please submit completed forms to the Student Leadership Development office in Room 8025.

To maintain active student club or organization status:

  • At the beginning of each academic year, student organizations are required to submit the "Student Organization Continuance of Status" form  and  "Charterd Club Document" form in order to remain an active organization on Sinclair's campus. This form must be completed and returned to the Office of Student Leadership Development (Room 8025) no later than October 31st before 5:00 p.m.
