  Incentive/Payment Request Form – PD # 1

  1. The form is used to request an Incentive/Payment only.
  2. This form is completed at or after attending a PD event.
  3. A director or coordinator does not need to sign PD # 1.
  4. Submission of a PD # 1 does not automatically result in payment.

Reimbursement Request Form - PD # 2

  1. This form is used to request a Reimbursement only.
  2. Please contact the SWRC office prior to the event for which a   reimbursement will be requested so that eligibility for financial support can be verified. 
  3. Complete the form after the event and attach any original receipts that are required.

Alternative Delivery Incentive Payment Request Form – PD # 1A

  1. This form is used to request an Incentive/Payment for an alternative professional development activity and is submitted upon completion of the alternative activity.
  2. This form requires the answering of questions both by the participant and his/her director prior to submission to the Resource Center for payment.
