Employee Medical Statement for Child Care Centers an Type A Homes (JfFS 01296 Rev. 9/2011) Click on Form Number and enter 01296 then click Search. You must include attached documentation of having received the MMR and Tdap vaccines with your medical statement.

A copy of your high school diploma, GED, or high school transcript.
It is recommended that the documents contained within the packet be completed no more than one month prior to the beginning of the semester for which you are enrolled.
Dates for packet submission will be posted on this webpage one week prior to the start of each semester. Packets will only be accepted on the dates indicated.
