Students in the OTA Program will be working in clinical sites (Directed Practice) during their first year. Most clinics require health information, including up-to-date immunizations, from students prior to working within their facility.
Once accepted into the program, students are required to perform physical activities in preparation for their future role as an OTA. For this reason a Personal Health History form must be completed by the student upon acceptance into the OTA Program.
Once students are accepted into the OTA Program, they will be provided with the Student Health Certificate form to be completed and signed by a licensed physician.
Students are given ample time (10 weeks) to complete and return these forms prior to entering the program.
