Removal of a Student for Unsafe/Unethical Practice

A student may be removed from a course by an instructor, and assigned an "F" grade for the course, for engaging in unsafe or unethical practice(s) as related to approved course/program requirements in college laboratory/workshop, clinical, practicum or internship settings. Such removal and grade may occur at any time during the term provided:

1. At the beginning of the course or quarter, the student was given written notice
    of required safety and ethical procedures as determined by department faculty
    and approved by the division dean;
2. The student was given instruction in the required safety and ethical practice(s);
3. The student was either found to be knowledgeable and competent in an
    assessment of understanding the required safety and ethical practices, or after
    having received appropriate support services (coaching, tutoring and/or other
    educational support services) repeatedly fails to demonstrate adequate
    knowledge and/or competency in the required safety or ethical practices; and
4. The unsafe and unethical conduct is sufficiently serious to warrant an "F"
    grade, even if the student were allowed to finish the course.
