Prior to the Friday, November 16th Girls in IT conference, all participants must complete this survey to be eligible for door prizes and to receive their goody bag.  The survey can either be printed or submitted online.  Please keep in mind the following information:

1.  If you choose to print the survey, please give it to the teacher who will accompany you to the event. 

2.  If you choose to complete the survey please complete in no later than Wednesday, November 14th at 3:30pm.
