Students who wish to take part in Sinclair's Service Learning Program must first fill out the Student Agreement Form. If you select your project online (click here to select project) you will be prompted to complete the electronic version of the Student Agreement. If your approved (by faculty or David Bodary, Service Learning Faculty Associate) Service Learning project/activity is not listed online, you must complete the agreement form, print it, and submit to Room 7L02.
For those already enrolled in Service Learning, the following form is available: Service Learning Site Evaluation Hours Log. This form is optional, however, it is recommended that it be completed to include in the student portfolio. It is not necessary to forward a copy to Service Learning.
