Service Learning Forms

Student Forms

  1. Students who wish to take part in Sinclair's Service Learning Program must first fill out the Student Agreement Form.  If you select your project online (click here to select project)  you will be prompted to complete the electronic version of the Student Agreement.  If your approved (by faculty or David Bodary, Service Learning Faculty Associate) Service Learning project/activity is not listed online, you must complete the agreement form, print it, and submit to Building 7, Room L02.
  2. Students are encouraged to fill out the Service Learning Project Confirmation/Feedback Form and submit it electronically upon completion of the Service Learning Project.
  3. For those already enrolled in Service Learning, the following form is available: Service Learning Site Evaluation Hours Log. This form is optional, however, it is recommended that it be completed to include in the student portfolio.  It is not necessary to forward a copy to Service Learning.
