Student Forms
1.	Students who wish to take part in Sinclair's Service Learning Program must first fill out the Student Agreement Form. If you select your project on line (click here to select project) you will be prompted to complete the electronic version of the Student Agreement. If your approved (by faculty or Marilyn Rodney, Service Learning Coordinator) Service Learning project/activity is not listed on line, you must complete the agreement form, print it, and submit to Room 6141.
2.	Students are required to fill out the Service Learning Project Confirmation/Feedback Form and submit to your professor upon completion of your Service Learning Project.
3.	For those already enrolled in Service Learning, the following form is available: Service Learning Site Evaluation Hours Log. This form is optional. However, it is recommended that it be completed to include in the student portfolio. It is not necessary to forward a copy to Service Learning.

1.	Service Learning Tracking & Evaluation Form Required by Sinclair Community College Service Learning. Return completed form to Marilyn Rodney, Room 6141 or email ,marilyn.rodney@sinclair.edu Please include action photos of your project.
2.	Faculty Survey Required by Sinclair Community College Service Learning. Return completed form to Marilyn Rodney, Room 6141 or email marilyn.rodney@sinclair.edu
3.	NEW! When your students turn in their Service Learning ProjectConfirmation/Student Feedback Form, after you have reviewed it, please send a copy to Marilyn Rodney, marilyn.rodney@sinclair.edu
